Birth & Death Certificates
All birth and death certificates occurring in Wasco and Sherman counties
are filed by Vital Records at the Health Department. Birth and death certificates
for those who are eligible to order can be obtained at the Health Department
for up to six months. Certified copies are $20 for the first copy and $15
for each additional copy at the time of the order.
Those who need to obtain certified copies after the six-month period may do so by mail using an order form which can be obtained at the Health Department. If a money order is enclosed with the order, the request should be filled within a week. Birth and death Certificates can also be ordered by phone from the Vital Records office in Portland, (971) 673-1190. You must have a credit card to make the order and there is an additional $10 fee for quick service. Orders are mailed the next working day unless there is a problem. You may also get a record by going to the Vital Records section in person at the Oregon Health Division, 800 NE Oregon St., Suite 205, Portland.
